Application Form

This application form must be submitted before June 30, 2003.

Important note:
It is very hard to change the data in your file after submittal. We suggest that you first read the entire form carefully. Then prepare all the requested information. Finally, fill in the form, check it, and submit it. You will receive automatic confirmation of your application by email. Please do not anger our applications administrator!

For your application to be considered, we need by the deadline to have received two letters of reference. You may however fill the application form without declaring at that time your referees. You can add them on later, using the updates form.

After you have submitted your application you will receive, by e-mail, instructions on how and where three referees should send recommendation letters on your behalf as well as on how to send additional material for your application. Please keep this message because you may need some of the data in there (like passwords) in the near future. It is your responsibility to contact them directly and verify that they have received instructions from us and that they are able to follow them. If they are not then you can instruct them to submit their letter by email to the applications administrator.

Please fill in all applicable fields of this application form! Fields marked with (*) are compulsory. In case you need to upload more files and/or add more referees you can use the updates form after you submit your application. Changes in other fields is difficult and possible only by contacting the applications adminitrator (who gets quite angry with trivial mistakes). It is especially important to enter the correct emails both for you and your referees.
